MYDSV - HOW TO REPORT A CLAIM – June 24, 2020 Please enter the following information when registering your claim: DSV country/region and division that is being claimed Shipment number (DSV Shipment ID) related to the claim Reference: Your own internal reference Claim type: DSV Cargo Insurance (shipments where insurance has been purchased from DSV, including DSV Protect) or Freight Forwarders Liability Claim sub-type: Damage, total loss, DSV Protect, etc. Date of loss: Date when the goods were damaged Claimed amount: Enter the amount claimed towards DSV and state which currency Damaged goods in kilos: Enter the net weight of the damaged goods Commodity: State which type of goods has been damaged Customer name: Name of the company making the claim Description: Describe what has been damaged and other relevant circumstances Attachments: Add attachments related to the claim such as pictures of the damaged goods, commercial invoice and POD. Finally, submit your claim It is only possible to edit the information in the form before the claim is submitted. Once the form has been submitted, it is no longer possible to edit the provided information.
